Stronger with less

Date January 7, 2009

There is an unspoken rule that you should never write what is impolitely called “knocking copy”. This is an article that compares and contrasts one product with another, making the other look really bad. We don’t do that here. We are always absolutely fair and never do more than state facts, not opinions. So here is something you can check out for yourself. If you look at the way in which the little blue pill is packaged (can’t think which drug that is), you’ll see it comes in 25mg, 50mg and 100mg. Most men seem to do best with an average dose of 50mg. Levitra, on the other hand, comes in 2.5mg, 5mg, 10mg and 20mg. Most men seem to do best with 10mg tablets but many get perfect results with the lower dosages. Why should anyone care? Well, you’re taking five times less of the same active ingredients to achieve the same results. As a general rule in the world of medication, the smaller the dose, the lower the risk you will experience any side effects. Do many people have side effects? Not that many with either drug but, if you were looking for a reason to prefer Levitra, safety would be top of the list for most.

Comparing policies saves you dollars

Date January 7, 2009

This last few months has seen all the prices of basic necessities rise. As jobs have come under pressure, the purchasing power of the average household has dropped. Nowhere has the family budget come under greater pressure than with health insurance. All too often, the premiums have been raised (again). This forces yet another tense discussion. Are families to gamble with the health of their children or can other savings be found? Sad to say, this discussion is no longer restricted to low-income families. A significant number of middle class families are also being forced to make ever more difficult decisions. Decisions to delay diagnosis and treatment until the sickness can be classed as an emergency and justify a hospital visit. If people are to stay insured, they must accept the best terms they can afford. Fortunately, online sites such as this allow people to get comparative information from multiple health insurance companies. Making the choice from the maximum possible number of quotes gives the best chance of savings. So how should you approach this task? 1. Before you start, write down a list of all the features you would like to see in your ideal policy. This gives you a shopping list to price as you go along. It will almost certainly be too expensive, but it gives you a good starting point. 2. Always compare quotes on like-for-like policies. If you use several sites to get the maximum spread of quotes, keep notes to ensure you use the same basic set of information about the policy you are seeking, the level of deductibles accepted, and so on. 3. Never make a decision purely on the premium quoted. Although this is the headline you see first, the devil is in the detail of each policy. You have to be determined and read through all the terms (even the small print). It’s vital that you get a clear picture of what is included and excluded, and see what conditions you have to fulfil to make a claim. Even more important is whether you have a right to renew the policy if you make a claim or you are found to have a disorder or disease that is going to be expensive to treat. There is nothing more devastating than to be diagnosed with a chronic illness and then find your insurance premium hiked up to unaffordable levels or renewal declined. 4. If there is anything you do not understand, ask an agent. Before you accept a quote, insurance companies are helpful and explain things. If you delay asking until you make a claim, this only leads to disappointment and dispute. Take the decision to buy on the basis of the best available information. By following this simple set of rules, you’re taking more positive control of your future, and there’s nothing more important than health to give you peace of mind.

Is it a social conscience or self-interest that should motivate us to dispose of medications safely?

Date January 5, 2009

It is fascinating to browse around the internet. There is always somebody thinking something interesting somewhere in the world. All you have to do is to find him or her. Take just one story from California as an example. It raises the difficult question of how you should dispose of “pills” you no longer need or which have expired. You could, of course, get in your car and drive down to your local pharmacy. Many offer a service to dispose of old and unwanted medications for you. But, the majority of us probably find the effort involved a deterrent. Why go to so much effort when you have a dumpster just outside your door? Or, if even that is too much effort, there is always the option to flush the problem away. Who would know? Who cares anyway? Do you ever wonder what happens after casual disposal? Your local waste management authority comes round to collect the refuse which is then dumped. There is little or no effort to sort the waste. Most authorities simply drive to the nearest landfill site and tip each load on to the growing mound of other rubbish. This pile then rots down as rain washes through it so, sooner or later, dissolved drugs end up in the watertable and potentially get recycled into your drinking water. The medications flushed go more directly into the water supply. So here is the worry. The rivers downstream from you supply water to the local towns and cities. That water supply contains what the experts call a “sub-lethal” cocktail of antibiotics, sedatives, painkillers, hormones and whatever else you so casually threw away. Perhaps you have no interest in the people downstream of you. I wonder what the people upstream think of you. But, back to Ambien. Ambien is, of course, a nonbenzodiazepine hypnotic. I am never reassured by the prefix “non”. In fact, Ambien works in exactly the same way as a conventional benzodiazepine and is probably just as addictive. For this reason, Ambien is listed by the Drug Enforcement Agency (DEA) as a controlled substance in Schedule IV. You will be pleased and delighted to know that the US Government has your interests at heart. It always wants to protect you and the environment (”always” is probably a slight exaggeration). State and Federal regulations limit the handling and disposal of controlled substances to DEA-authorised individuals and organisations. The DEA’s Office of Diversion Control aims to prevent the diversion of legitimately manufactured (or used) controlled substances into the illegal drug traffic. If there is no-one else immediately available to handle the disposal, the controlled substances should be collected by a law enforcement officer. So, if your local pharmacy has not registered with the DEA, their only way of disposing of your unwanted drugs is to call the cops. No wonder they looked so pleased when you asked. But San Mateo County, California has placed collection boxes inside the entrance halls of eleven police departments. Anyone can walk in and leave their unwanted medications (including Ambien but excluding all illegal substances) and walk out - no questions asked. And is this a welcome service? Over the first fifteen months of the program, local citizens have deposited 1,800 pounds of medications (not all Ambien, of course). So there is clearly a demand for this kind of service. It is pure self-interest, of course. Who wants to get high from drinking tap water? Hmmm. Wait, that is not quite the right question. How many men want to take female hormones - I am sure breasts would be alright. And do we really want all those bacteria out there to get used to all those antibiotics in the water? If you don’t know the answers to these and other questions of social conscience (or self-interest), take an AmbienAmbien and sleep on it.

How to deal with anxiety

Date January 5, 2009

For once, everyone has a good reason to feel stressed and anxious. The economy is in trouble. Before the housing bubble burst and the credit crunch came, all we had to worry about were the “pressures of modern living”. We were all running fast to pay for what we needed, but we could tell ourselves it was all worth it by seeing our efforts at work rewarded by those regular checks, looking at our more comfortable homes, and enjoying our family life. Now, we worry about whether we can keep our jobs and stay in our homes. We all seem to be more bad-tempered and relationships are not so good. As the pressures build, it’s easy to feel despair or lose hope. Anxiety begins to interfere with our daily lives and the threat of depression edges closer. If you ask your local healthcare professional for help, he or she is also feeling stressed in these conditions. The ideal would be a discussion about your situation and advice. But physicians have reduced appointment times to fit more patients into a working day. Thus, the answer is likely to be a prescription for xanax. As the best of the anti-anxiety drugs, it can quickly help you to feel better. Unfortunately, good though xanax is, a tablet cannot resolve your financial circumstances. You may feel good for a few days, but your problems are still there waiting to be addressed. The “cure” for economic troubles is positive thinking and action to defend your position against threats. Keeping this real, the ideal would be never to use xanax. Do not be deceived by the ease with which you can buy xanax online. With the power of your mind, you can devise coping strategies. By trusting in yourself, you can keep your mind focused and positive. It becomes a self-reinforcing way of rebuilding your life. But not everyone can do this unaided. If the people around you are not supportive, using xanax in a short burst can give you a respite, relief from the sense of panic that threats to overwhelm you. If you use this time to restore your self-confidence, you can go on with your life without ever having to use xanax again (or, if you relapse, you can buy xanax online for use in another short burst). Following this strategy will give you a way out of the despair without running the risk of dependence on a tablet to keep your going.

What does the US Government actually say about generics?

Date January 5, 2009

It is always pleasing to find a government agency with a specialized website. In this case, the FDA maintains a site devoted to generic drugs at http://www.fda.gov/cder/ogd/. There are over two-hundred-thirty staff whose task it is to ensure that the generic drugs approved are safe for consumers to use. Just to give you an idea of the scale of the operation, in 2004, there were 563 applications made to license the production of a generic drug. There were 880 applications made in 2007. In other words, a vast number of generic drugs come on to the market every year. They are in every way as safe and effective as the original branded version they now compete with. So generic viagra is identical to branded viagra except that it is cheaper. As a part of its function, the Office of Generic Drugs in the FDA monitors and researches the way in which the market works when generics enter. The supposed advantage is that any competition from a generic manufacturer will force the holder of the patent to drop its price. The consumer therefore benefits immediately. But its research shows that one generic manufacturer has very little effect on prices within the market. The FDA has a detailed analysis of the market from 1999 to 2004 and price is not significantly affected until a second generic manufacturer starts to sell into the market. The reason is simple. When the first generic competitor enters, even a small price reduction gives it a market edge. But when a second company appears, the two generic manufacturers compete with each other to set the price for a generic version of the original drug. The average result is half the original branded price. In fact, if you now look at the prices for generic viagra you will find it is often less than half the price of the branded version. The greater the quantity you buy, the better the price per tablet (plus the possibility of bonus tablets from some of the online pharmacies). This is a perfect working example of the research in action. It is also a perfect explanation for Big Pharma’s dislike of generic manufacturers. Once two generic manufacturers start competing with each other, the original manufacturer’s profit margin drops like a stone. It’s great for the consumer. Bad news for the stockholders.

Talking about the diseases no-one wants to talk about

Date January 5, 2009

For some reason, there’s a reluctance to talk about the sexually transmitted diseases (STDs). In a country where abstinence before marriage is one of the main measures to prevent STDs, actually talking about these problems is perhaps a little difficult. But, for once, we’re going to break with convention and talk about one of the these diseases and the standard form of treatment. Chlamydia is the most common bacterial infection, mainly affecting younger people. You are at risk if you have multiple partners. The problem is that, in most people, there are no immediate symptoms and most are completely unaware that they have caught the disease. The bad news is that failure to get treatment once the symptoms do become apparent can lead to serious complications. So let’s get the ST element out of the way. You catch this disease by having conventional sexual intercourse, although it can also be transmitted by both oral and anal sexual activity. The symptoms to watch out for are fairly obvious. In men, you will find it painful to pass urine and/or you will find a discharge coming from your penis. In women, you will feel a burning sensation when urinating and/or produce a vaginal discharge. In both men and women, a failure to get treatment can cause infertility. However, if you have been engaging in oral or anal sex, the bacteria responsible can simply sit there without producing any symptoms. This leaves you as a carrier and the next time you have oral or anal sex, you can pass the bacteria on to your next partner. If there are no symptoms, the only sure way to test for the condition is through swabs. In men, a urine test is not unreliable. The presence of the bacteria cannot be diagnosed from a blood sample. The standard form of treatment is the use of an antibiotic such as zithromax. Under normal circumstances, a single dose can be sufficient to clear up the problem. Some people find they experience some nausea and stomach cramps after taking the dose. This usually passes quite quickly. However, you should always wait at least one week before having sexual intercourse again. If your partner is also treated (which is a sensible precaution), you should both wait a week until resuming sexual activity. The final thing to note is that zithromax affects the way in which an oral contraceptive works. There is an increased risk of pregnancy if you resume sexual activity before the zithromax is completely clear from your system. Using a condom is desirable until you are sure pregnancy cannot result.

How to treat men diagnosed with cardiovascular disease?

Date December 29, 2008

Because diagnosis and treatments have been improving steadily over the last decades, more men than ever before are now living longer with cardiovascular disease. In the last century, heart attacks were the most common cause of death by a significant margin. Now with better drugs and lifestyle changes, deaths from cardiovascular disease have been falling. But this does not mean that these men have a satisfactory quality of life. The majority of men are now older, overweight, living in deprived areas and depressed. Many also find themselves affected by Type 2 Diabetes as the weight increases. It’s difficult to prevent this development because, with limited budgets, most men cannot afford healthy food options. Cheap processed food has a high fat content and excess salt. General risks are increased significantly if the men also smoke and neglect to take any physical exercise.

As a routine, men should be encouraged to monitor their waist measurements as a simple means of judging when the risks are rising. Anything over 37 inches for men (and 31.5 inches for women) is in the danger zone and suggests the need to check blood pressure. Once a person is at risk, there should be positive intervention to counsel lifestyle changes, starting with quitting smoking and eating a more healthy diet to reduce the cholesterol levels. However, there is a significant link between cardiovascular disease and erectile dysfunction (ED). It may be a side-effect of the drugs to control the cardiovascular problems, damage to the arterial system or nerve endings, or it may be linked to the depression many experience. The interaction between these factors can produce a vicious cycle. As the ED worsens, depression deepens justifying more powerful drugs that further affect sexual performance.

All men at risk should be asked a direct question, “Are you suffering from ED?” If the answer is, “Yes.” this should trigger a review of the drugs being taken, and the prescription of viagra to begin restoring sexual activity. However, to be effective, men must be convinced that ED is a symptom of cardiovascular disease and be prepared to answer the question honestly. Too many consider such a question to invade their privacy, and either lie or refuse to answer. In its own right, this process is difficult because those who live in poverty and in deprived areas are often not contacted as easily and are reluctant to come into clinics for any type of treatment. It may be necessary for local health providers to physically go into the community to provide health screening and health assessments. This is preventative medicine as it should be practised. The earlier ED is identified as a predictive symptom of cardiovascular disease, the more men may be saved the trauma of a stroke or heart attack. This will require a diversion of funding to community-based medicine and to state funding of medications such as viagra to help treat the ED. But it should save money in the long term.

Age is no barrier

Date December 29, 2008

Life is never fair. Even though the majority may avoid some of the worst possibilities, there are always the few who seem to have the worst of luck. Look around the media and it’s impossible to avoid the stories. Sometimes, it’s news of accidents and crimes where we are shown the victims and their suffering. Sometimes it’s the way of collecting charitable donations. Images of young and old are everywhere reminding us that there will always be some who need our help. One of the ways in which we prove we are human is through empathy. By our willingness to put ourselves in the position of others and to imagine what it would feel like to have their problems.

For both men and women, there are hormones and biological drives which guide physical growth and give us an interest in reproduction. Because of the physical gender differences, boys and girls are nurtured to have different functional expectations. Being a man means fertilizing the eggs. Being a woman means having the resulting babies. In later life, should something happen to interfere with reproductive abilities, those affected can at least look back on some life where pleasure and function worked together. But it is emotionally difficult when nature or an accident denies you the chance to experience sexual activity. Although those affected never know what they have missed, there is an underlying sadness and frustration.

For teenage boys, erectile dysfunction (ED) is not a common problem. As a result, there is little research. Most healthcare providers prefer a simplistic and dismissive diagnosis of psychological dysfunction and, where there is medical insurance to cover it, refer those affected for counseling. Such research as there is actually prefers formal assessment of the vascular system. A recent meta-research study examined research carried out between 1998 and 2003, looking for common factors in diagnosis and treatments for teenagers under the age of 19 years. On average the young men had suffered symptoms of ED for an average of two years with the most common causes being perineal and penile trauma, and surgery. Formal vascular examinations were carried out in 60% of all cases and almost half were found to have serious problems justifying further angiographic study or surgery.

In other words, half the teenagers given a formal test were found to have abnormal blood flow as the explanation for their ED. The practice of routine referral for counseling is expensive and ineffective because, in most cases, the usual consequence of recommending viagra will not represent the best treatment. Indeed, because of the expense, most healthcare providers simply write out a prescription for viagra and send the patients away with the reassurance that they will grow out of it, or everything will work out fine when they meet the right woman. At the very least, there should be a detailed survey of medical history to see whether there is evidence of traumatic injury as the trigger for haemodynamic testing. Our young always deserve the best treatment.

A New Drug to Fight Cholera Epidemics

Date December 29, 2008

A recent medical study held in Bangladesh investigated the effectiveness of traditional cholera treatments and possible alternatives. The problem of cholera is very complicated in third world countries where the water supplies can be the source of numerous bacteriological diseases. Apart from the aspect of water supplies another problem, which contributes to the development of cholera epidemics in poor countries, is that traditional medication gradually loses its effectiveness with bacteria developing resistance to it. In such situation finding a new drug for treating cholera is crucial. Having these goals the study compared Cipro (ciprofloxacin), which is commonly used in cases of severe cholera for over a decade, and a macrolide antibiotic. The study involved 195 men suffering from severe cholera, divided into two groups. The first group was given Cipro and the second - macrolides respectively. The results where impressive and gave base to an intense discussion about the effectiveness of medicine used for cholera treatment today as well as the possibility of using macrolide antibiotics in such cases. Cipro, which only a few years back was effective in a single dose against cholera, failed in ¾ of the patients in the study. This can be explained by the fact that Vibrio cholerae - the bacteria causing cholera - was able to rapidly develop tolerance reaction to Cipro, rendering it ineffective with time. In contrast, macrolide antibiotic treatment has shown significant results with 73% of the patients being cured with a single dose. Furthermore, the group treated with macrolides has had illness symptoms manifested only for 30 hours after taking the medicine in comparison to more than three days in case of Cipro. Although, the studies were held on men only, there’s no reason to believe the results would be different in case of women. The results of this study have displayed the importance of developing new drugs which would be as effective as Cipro was a decade ago. Macrolide antibiotics used in the study seem like a great alternative to the traditional medication. There are numerous generic variations to this class of drugs, like Zithromax, which are available all over the world. But still, the doctors argue that a single drug will not solve the problem of cholera, because it is primarily caused by poor sanitation and water supplies. The bacteria may develop resistance to Zithromax with time too, like in case of Cipro, thus it can’t be regarded as a constant solution to the problem. The radical changes concerning sanitation and water supplies, which Europe and America have undergone a century ago, should be taken in developing countries too, but that seems like a much more complex solution compared to medications.

Alternative Treatment of Otitis Media

Date December 29, 2008

A new study on the ways of treating otitis media which has taken place in one of medical institutions of New Orleans has compared two popular medications for treating the condition and has found some interesting results. Dr. Stephen Eppes of the Alfred I. duPont Hospital for Children in Wilmington, Del, has stated that this study has opened a whole new perspective for treating otitis media and might be regarded as an alternative to the methods used today. The results of the study have already been submitted to the Food and Drug Administration for further investigation and approval, and if the results are positive a new drug may be added to the list of recommended medications used for treating otitis media in children. The studies have displayed that after a 12 to 16 day period two different antibiotics from different drug classes where equally successful with approximately 87% of children having no symptoms of otitis media after the course. A repeated examination in two weeks after the initial course has shown that about 75% of children were completely cured with a few cases of reclusion. In what concerns side-effects, the new drug has shown better results with 17% compared to the standard medication with 23% of children having symptoms such as diarrhea and rash. The first drug has caused only diarrhea as a side-effect. The children taking part in the study have had no history of antibiotic intolerance or failed treatment with antibiotics and were generally in a better condition than the patients who are subjected to tympanocentesis. Researches have thus concluded that both Azithromycin (the new drug) and amoxicillin/clavulanate (the standard medicine) have approximately the same level of effectiveness in the long run, with the first on being more effective in the initial stage of the disease. Azithromycin is better absorbed by the tissue and has longer half-life of about 60 hours. This makes a single dose of the drug as effective as a 10-day course of amoxicillin/clavulanate. Besides, the new drug can be used with children who have had an allergic reaction or intolerance to penicillin-based antibiotics. Good news for those, looking for an effective treatment of otitis media.
